What Is Road Rash?

Road rash is technically a type of burn known as a friction burn. This injury occurs when the skin is forcefully scraped over an abrasive surface, such as asphalt. This leads to a combination of heat burns (caused by friction) and open wounds (abrasions) on the skin. Friction burns are commonly referred to as “road rash” because they are very common in motor vehicle accidents, particularly bicycle and motorcycle accidents in which victims are often ejected from their vehicles.

Road rash can range from relatively mild to severe and most commonly occurs on arms, elbows, knees, shins, and ankles. Although many motorcyclists wear heavy, protective clothing, gloves, boots, and other gear to reduce the risk of road rash, these protections are not always sufficient in preventing serious injury.

Like other types of burn injuries, friction burns are categorized according to severity:

  • First-Degree Abrasions: First-degree abrasions are considered “mild” injuries and generally only affect the upper layer of skin (epidermis). First-degree abrasions do not bleed and can typically be effectively treated at home.
  • Second-Degree Abrasions: Second-degree abrasions are classified as “moderate” injuries. They may involve damage to the upper and lower layers of skin (epidermis and dermis, respectively). Second-degree abrasions may result in mild bleeding.
  • Third-Degree Abrasions: Third-degree abrasions are severe injuries. Also known as “avulsion wounds,” third-degree abrasions typically involve significant tearing of the skin and damage to deeper tissues, as well as considerable bleeding.

Moderate to severe road rash requires immediate medical attention, especially when abrasions cover large areas of the body or occur on the hands or face. Without adequate medical intervention, road rash can lead to serious complications, including potentially life-threatening infections.
